This Business Associate Agreement (“Agreement”) is entered into by and between McKesson Specialty Care Distribution, LLC d/b/a McKesson Specialty Health (“MSH” or “Business Associate”) and the Covered Entity named in the Regimen Profiler Account (“Practice”) and is effective as of the date indicated below. MSH and Practice may be individually referred to as a “Party” and, collectively, the “Parties” in this Agreement.
