This Class Action Settlement Agreement (“Agreement”) is made this 2nd day of December, 2016 between Plaintiff Yolanda Goodman (hereinafter “Plaintiff”), on behalf of herself and members of the certified class as defined herein, and the following defendants: Eugene K. Cashman, III.; Jamie R. Carter, Jr.; David G. Baytos; David Raines, Jr; W. Brad McCormick; Jason W. Collard; Herschel F. Owens; Andrew Luttrell; Keith M. Ingram; Randall Catt; William Johnson; Lannie L. Lancaster; Julio P. Ruiz; Sherry L. London; Ness S. Sechrest; Randy R. Sullivan; Leven Williams; Cigna Health and Life Insurance Company; and Crittenden Hospital Association (hereinafter collectively referred to as the “Defendants”).
