EQUIPMENT LEASE AGREEMENTEquipment Lease Agreement • July 27th, 2016 • Ohio
This Agreement is between MCR Medical Supply, Inc. of 3341 Centerpoint Drive, Suite C, Grove City, Ohio 43123 (“Lessor”) and Lessee under the terms specified herein and in attached Exhibit A. The Lease begins on the Term specified above and Lessee shall pay, by credit card only, the Rent and Required Deposit (“Deposit”) before taking possession of the Equipment. Lessor will refund the Deposit to Lessee at the end of the Term provided that Lessee has performed all of the Lessee's obligations under this Agreement.
