This Maternal Data Center Participation Agreement (the “Participation Agreement”) is entered into as of _______________________ (the “Effective Date”) by and between ________________________________, (the “Participant”), and the Comagine Health, a Washington non-profit, 501(c)(3) tax-exempt corporation (“Comagine Health”), (individually, a “Party” and, collectively, the “Parties”).
