Participant Agreement and Waiver – Team CaptainParticipant Agreement • January 31st, 2022
Team Captain Responsibilities: Team Captains are hereby responsible for the collection and completion of his own players and their corresponding participant agreement and waiver forms. This includes, but is not limited to, having all players on your team complete their own individual waiver, keeping all waivers with you in a folder when playing on the sideline (preferably in a folder), and in the event of one of your own players no longer able to participate in that season, you can replace that team member with a new member, but are still responsible for having the new member complete a participant agreement and waiver form. Walnut Creek Flag Football will assume no liability for team captains negligence in regards to waivers not completed or collected. This is the responsibility of the team captain. This means that at any time a new player is added to your team, anytime between the beginning of the season through the championship, it is the team captains responsibility to keep all wai
