The individual named below (referred to as "I" or "me") desires to enter the premises of Community Swim Club of O’Hara Township (“CSC”), located at 106 Aqua Drive, Pittsburgh, PA 1238 (the "Premises") to engage in swimming and other recreational activities (the "Activities"). As lawful consideration for being permitted by CSC to be on the Premises and engage in the Activities, I agree to all the terms and conditions set forth in this agreement (this "Agreement").
