THIS MASTER PROFESSIONAL SERVICES AGREEMENT (the "Agreement") is made and entered into the 2nd day of September, 2015, by and among the Village of Westmont (“VILLAGE") and PUSH WELLNESS SOLUTIONS, INC. ("PUSH"), a Delaware Corporation. (The VILLAGE and PUSH are individually referred to herein as the "Party" and collectively as the "Parties.")
