This Agreement for Electronic Access to Protected Health Information (PHI) hereinafter “Agreement” is entered into between (hereinafter “Outside Entity”) and the University of Washington (hereinafter UW Medicine). The University of Washington is a hybrid entity and has designated its healthcare components and non-healthcare components as described in COMP.101 Patient Information Privacy and Security Compliance Program and Administrative Requirements.
