Parent Release: I hereby grant permission for the below named to participate in all camp activities. I hereby authorize the staff of The Eagles Golf Club of Tampa Bay to act for me accordingly to their best judgement in any emergency medical attention, and hereby waive and release The Eagles Golf Club of Tampa Bay and all their employees and agents from all claims on account of any injuries or illness sustained by my child while attending the above said Junior camp.
