THIS EMPLOYMENT AGREEMENT (“Agreement”) is made and entered into on the last date written below, by and between HEARTLAND COMMUNITY HEALTH CLINIC, an Illinois not-for-profit corporation, and Federally Qualified Health Center (FQHC) located and doing business in Peoria, Illinois, sometimes hereinafter referred to as “HCHC” and ________________ (hereinafter referred to as “DENTIST”).
