This Client Agreement (the “Agreement”) is entered into on this day of , 2022 (“Effective Date”) by and between Spring Grove Area School District, a public school district located in Spring Grove, York County, Pennsylvania (“Client”), and Everside Health, LLC, a Delaware limited liability company (“Everside”) and Paladina Health Medical Group, PC (“Everside PC”). Client, Everside, and Everside PC are each individually a “Party” and collectively the “Parties” to this Agreement.
