This Provider Agreement (hereinafter "Agreement") is made and entered into by and between Anthem Health Plans of Kentucky, Inc. d/b/a Anthem Blue Cross and Blue Shield (hereinafter "Anthem") and Mercer County Schools (hereinafter "Provider"), effective as of the date set forth immediately above Anthem's signature (the "Effective Date").
