THIS AGREEMENT is entered into between Area Agency on Aging of Palm Beach/Treasure Coast, Inc. (Agency) and (Provider Name), hereinafter referred to as the “Provider,” and collectively referred to as the “Parties.” The term Provider for this purpose may designate a Vendor, Subgrantee or Subrecipient.
