Professional Services Agreement Between the County of San Mateo and Larry K. Fan, M.D.For Plastic Surgery Services THIS PROFESSIONAL SERVICES AGREEMENT is entered into by and between the County of San Mateo, San Mateo Health System (“County”) and Larry K. Fan, M.D. (“Contractor”). W I T N E S S E T H: WHEREAS, County operates health care facilities collectively known as “San Mateo Medical Center” (SMMC); and WHEREAS, it is necessary and desirable that Contractor be retained for the purpose of performing professional services described in this Agreement for SMMC; and WHEREAS, pursuant to Government Code Section 31000, County may contract with independent contractors for the furnishing of such services to or for the County; and WHEREAS, Contractor desires to provide such services all upon the terms and conditions stated below, and this Agreement is entered into for the purpose of defining the parties’ respective rights and responsibilities.
