THIS ATHLETE AGREEMENT, effective as of (the “Effective Date”), is by and between USCA, a nonprofit corporation having its principal office at 5525 Clem’s Way, Stevens Point, WI and the athlete (Print Name) signing below (“Athlete”), whose address is also set forth below. Athlete and USCA may be collectively referred to herein as the “Parties” and each individually as a “Party.”
