This Statement of Work is entered into as of this 1st day of January, 2023, by and between GBS Benefits, Inc., a Utah corporation with offices at 2200 S Main St. Suite 600, South Salt Lake, UT 84115 (“GBS”) and San Juan County, a Utah corporation with offices at 117 South Main, Monticello, UT 84535 (the “Client”) pursuant to that certain GBS Master Services Agreement by and between GBS and Client (the “Agreement”). This Statement of Work is incorporated into the Agreement by reference. Any term not otherwise defined herein shall have the meaning set forth in the Agreement.
