This Financial Planning and Consulting Services Agreement (the “Agreement”), dated on this day of _, 20 , is between the undersigned parties (herein referred to as the “Client”), and Integrated Wealth Concepts LLC, doing business as Integrated Financial Partners, a registered investment advisor, whose mailing address is 300 Fifth Avenue, 3rd Floor, Waltham, MA 02451 (herein referred to as the “Advisor”). The Advisor shall provide the Client with financial planning and consulting services as indicated in Item 1 of this Agreement. This Agreement becomes effective on the date first written above.
