This agreement between the Utah Department of Health (Department), Office of Public Health Assessment (OPHA) and the Responsible Party is to disclose BRFSS data collected in partnership with the U.S. Centers for Disease Control and Prevention (CDC) and maintained by OPHA. This agreement establishes the terms and conditions under which the Responsible Party may use and disclose the data described herein.
