THIS MUTUAL TERMINATION OF EMPLOYMENT AGREEMENT (“Agreement”) is entered into on this 10th day of March, 2016, but effective as of the Effective Time (as defined below), by and among LAPORTE BANCORP, INC., a Maryland corporation (“LPB”), LAPORTE SAVINGS BANK, an Indiana state chartered savings bank and wholly‑owned subsidiary of LPB (“LPSB”), and PATRICK W. COLLINS, the current Senior Vice President/Mortgage Warehouse Lending of LPSB (the “Executive”) (hereinafter collectively referred to as the “Parties”). HORIZON BANCORP (“Horizon”), a bank holding company under the Bank Holding Company Act of 1956, as amended, and HORIZON BANK, NATIONAL ASSOCIATION, (“Horizon Bank”), a wholly‑owned national banking association subsidiary of Horizon, are Parties for the limited purposes described herein.
