This INDEPENDENT CONTRACTOR AGREEMENT (the “Agreement”) is entered into by and between VENDOR NAME, (the “Contractor”) and Northern Arizona Intergovernmental Public Transportation Authority, a political subdivision of the State of Arizona (“NAIPTA”), effective as of November 15, 2017 (the “Effective Date”).
