This Research Funding Agreement (the “Agreement”) is made by and between the Focused Ultrasound Foundation, a 501(c)(3) organization and public charity having its principal place of business at 1230 Cedars Court, Suite 206, Charlottesville, VA 22903 (hereinafter referred to as the “Foundation”) and Institution, an EDUCATIONAL INSTITUTION/CORPORATION having its principal place of business at BUSINESS ADDRESS (hereinafter referred to as the “Institution”), and PI_Salutation«PI_FirstName PI_LastName, PI_Credentials having a professional affiliation with Institution (the “Investigator”), and is to be effective as of the date of the last signature below (the “Effective Date”).
