This ACN Affinity Fundraising Program Agreement between ACN, Inc., a Michigan corporation with offices at 1000 Progress Place, Concord, North Carolina 28025 (“ACN”) and (the “Non-Profit”) with offices at is effective on the date of the second signature below (the “Effective Date”).
