Bryn Mawr Ave., Suite #700, Chicago, IL 60631, and Verizon North Inc., f/k/a GTE North Incorporated, Verizon South Inc., f/k/a GTE South Incorporated, a Wisconsin corporation (“Verizon”), with offices at 8001 West Jefferson, Fort Wayne, IN 46804, (each of Verizon and US Cellular being individually, a “Party” and, collectively, the “Parties”).
