I/We hereby request that my/our child(ren), , be permitted to participate in the Dinsmore Homestead Day Camp. I/We understand that these activities will take place on the property of the Dinsmore Homestead and that my/our child(ren) will be under the supervision of the designated camp personnel and Dinsmore staff on the dates specified. I/We release and agree to indemnify Dinsmore Homestead Foundation and its representatives from liability for any accident in which my/our child(ren) may be involved or any injury to my/our child(ren) which may occur in connection with the day camp. I/We consent to the conditions for participation in this activity.
