The terms and conditions incorporated in this order form (“the Order Form”) constitute an addendum to the Company’s Supply Agreement (Telecommunications Services Agreement, Switchless Resale Agreement, Carrier Services Agreement or Interconnect Agreement). Defined terms used that are not defined in this Order Form shall have the meanings attributed to them in the Supply Agreement and any terms of such Supply Agreement not expressly amended by this Order Form shall continue to have full force and effect.
