This amendment is issued to renew the current contract for an additional eighteen (18) months from 01/01/19 through 06/30/20, to remove the Procurement Contract Analyst information, to add two (2) agreed upon clarifications to page eight (8) of the contract, to add the 2019 school year assessment cost from 01/01/19 to 06/30/19 to page nine (9) of the contract, to add the 2019-2020 school year assessment cost from 07/01/19 to 06/30/20 to page nine (9) of the contract, to remove the Contractor Index information from page 10 of the contract, to repaginate the contract, and to update the name and contact information of the Contractor’s Contact.
