NAME OF LOCAL EDUCATION AGENCY (LEA) AGREEMENT NUMBER This is to Certify that: Each school listed on the Web Application Agreement is nonprofit and exempt from federal income tax under the Internal Revenue code of 1954, as amended, Section 501 (c) (3). No member of or Delegate to Congress or Resident Commissioner shall be admitted to any share or part of this agreement or to any benefit that may arise therefrom, but this provision shall not be construed to extend to this agreement if made with a corporation for its general benefit. The information supplied here in is true and correct to the best of my knowledge and that I have been authorized and directed by the governing body of the named LEA to enter into this agreement and to handle all negotiations with the named State Agency.
