This Athletic Trainer Agreement (the “Agreement”), dated , is by and between Excelsior Orthopaedics, LLP (“Excelsior”) with corporate offices located at 3925 Sheridan Drive, Amherst, NY 14226 and Williamsville Central School District (“WCSD” or the “District”) with offices located at 105 Casey Road, East Amherst, NY 14051 and affiliated schools of WCSD (collectively referred to herein as the “School” or WCSD). This Agreement shall supersede previous Athletic Trainer Agreements between Excelsior and WCSD.
