This Agreement is entered into by and between the School District of Volusia County, Florida, hereinafter referred to as the “LSD”, and the State of Florida, Department of Health, Volusia County Health Department, hereinafter referred to as the “CHD”. The purpose of this Agreement is to establish the terms and conditions to support the delivery of comprehensive school health services.
