I, , Volunteer (referred herein as “I”, “me”, or “my”) have chosen to perform certain volunteer services in connection with the County’s COVID-19 response efforts (“Volunteer Services”) without compensation for the County of San Mateo (“County”). I understand and acknowledge that I am 18 years of age or over and have legal capacity to sign this Volunteer Agreement, Waiver and Release of Liability, and I am not a County employee and I am not entitled to receive or accrue any benefits of employment including, but not limited to, payment, salary, wages, vacation, sick leave, workers’ compensation, health care, retirement benefits or insurance of any kind.
