This Agreement will be executed in counterparts – it will be signed separately by each participating organisation. Each counterpart shall be deemed to be an original document and all of the counterparts taken together shall constitute one single agreement between the participating organisations. A full list of participating CCGs and General Practices will be maintained by NHS Arden and GEM CSU.
