This second amendment is made to the existing HIPAA business associate terms and conditions, and any previous amendment thereto, agreed to by and between [NAME OF BUSINESS ASSOCIATE], hereinafter called “Business Associate” and the University of Pittsburgh Medical Center, hereinafter called the Covered Entity (the “T&Cs”). Business Associate and Covered Entity shall be referred to herein individually as a “Party” or collectively as the “Parties.”
