This DentalHQ Platform Customer Agreement (“Agreement”) provides information about how you can use the DentalHQ, LLC (together with its affiliates, successors, and assigns hereinafter referred to as “DentalHQ”, “our”, “we”, or “us”) Platform, as defined below, to create one or more dental membership plans (each a “Dental Membership Plan” or “Plan”) for your patients. To create a Dental Membership Plan using the Platform you must first enroll with DentalHQ at www.dentalhq.com, which includes completing the enrollment process for your practice(s) (“Practice Enrollment Form”) and agreeing to the terms of this Agreement. Upon enrollment, you will gain access to DentalHQ’s cloud-based software (“Platform”) and our Management Services, as defined below, which you may use to create, administer, and market one or more Dental Membership Plans for your patients.
