THIS HEALTH SERVICES AGREEMENT (this “Agreement”) is made and entered into to be effective as of November 7, 2018 (the “Effective Date”) by and between City of The Colony (“Client”), with principal offices at 6800 Main St, The Colony, TX 75056, and Marathon Health, LLC (“Marathon”), a Delaware limited liability company with principal offices at Champlain Mill, 20 Winooski Falls Way, Suite 400, Winooski, VT 05404. Client and Marathon may each be referred to in this Agreement as a “Party” and, collectively, as the “Parties”.
