I, the undersigned, parent or guardian of (Name of Child/Youth) , a minor under eighteen (18) years of age, expressly accept and agree to the following terms and conditions (hereinafter, collectively, the “Release Agreement”) in consideration for my child’s volunteer participation in the Cornell University 2023 Sheep and Goat Symposium program/activity/camp/event (hereafter called “Program”) at Cornell University campus located in Ithaca, NY from October 27, 2023 to October 29, 2023.
