This Brand Ambassador Agreement (this “Agreement”) is being provided in connection with your minor child’s participation in and activities with, which may or may not take place on your child’s school campus, of Positively Social, Inc., a California nonprofit public benefit corporation (the “Company”). If you agree to your child’s participation in the Company, please fill out the form below completely and clearly, sign it where indicated and send it to the Company as indicated below. For good and valuable consideration, the receipt and sufficiency of which is hereby acknowledged, you here acknowledge and agree to the following:
