This is a two party agreement between the named Practice and NHS Leeds to participate in the Leeds Risk Stratification project, implemented in the Leeds health community in collaboration Leeds North Clinical Commissioning Group (CCG), Leeds South & East CCG, Leeds West CCG, NHS Leeds, Leeds Community Healthcare Trust, Leeds & York Partnership Foundation Trust, Leeds Teaching Hospitals Trust and Leeds City Council Adult Social Care.
