The template includes the required Contract provisions for compliance with CACFP requirements. The Institution and FSMC must insert the required information into the Contract template, including information in Attachment B. Any Institution that has a Food Service Contract must submit the required information to the State agency before NC CACFP can approve the Institution’s CACFP application.
