This Business Associate Agreement (“Agreement”) is entered into by and between Valley View Hospital Association, a Colorado non-profit Corporation (“Hospital” or the “Covered Entity”) and City of Glenwood Springs (the “Business Associate” or “BA”) (collectively referred to herein the “Parties” or individually as a “Party”) to be effective as of the 4th day of November, 2020 (“Effective Date”).
