The purpose of this letter is to provide a written record modifying the Clinical Training Affiliation Agreement (“Affiliation Agreement”) between the Elson S. Floyd College of Medicine at Washington State University (WSU COM) and HOST AGENCY as stated in this Affiliation Agreement Implementation Letter (“Implementation Letter”). The Affiliation Agreement is incorporated herein and no other terms and conditions are changed except where expressly stated in the Implementation Letter.
