You will be deemed to have accepted your ALINE Card by ADP® Visa Prepaid Card (the “ALINE Card”), to have agreed to the terms and conditions of this Cardholder Agreement (the “Agreement”) and to pay the fees set forth in the short form and long form List of fees you received with your ALINE Card (collectively, the “List of Fees”) if you do any of the following: (a) request and receive the ALINE Card; (b) sign the back of the ALINE Card; (c) activate the ALINE Card; or (d) use the ALINE Card. Through the telephonic and electronic activation process, ADP (as hereinafter defined) provides you a process to voluntarily consent to receiving your wages on the ALINE Card and for you to confirm that your employer provided you alternate means to receive your wages. Please contact your employer for more information if you have not been advised of alternative means of receiving your wages. By electing to receive your wages on the ALINE Card, you authorize your employer to deposit your wages on the
