The Health and Human Services Commission ("HHSC") and Real Time Transport Corporation (“Provider”) having its principal office at 2206 Century Center Blvd, Irving, Texas 75090 (each a “Party” and collectively “Parties”) desire to further amend the Transportation Provider Agreement which was effective on November 12, 2015, as amended by Amendment No. 1 as of September 1, 2016, and Amendment No. 2 as of September 1, 2017, (collectively, the “Agreement”), with the terms and conditions contained herein (“Amendment No. 3” or the “Amendment”).
