This AFFILIATION AGREEMENT ("Agreement"), effective the 6th Day of January 2021 ("Effective Date") is by and between Boise State University by and through its College of Health Sciences, an institution of higher education of the State of Idaho ("Boise State" or "University"), having its principal offices located at 1910 University Drive, Boise Idaho, 83725 and Caldwell School District ("Facility"). Boise State and Facility are collectively referred to herein as the "Parties," each individually a "Party."
