Facility Use Agreement

This agreement shall become effective upon the signatures of representatives of both parties affixed herein and shall terminate at the date of last use as designated above. Any future use of said facility after said date shall require the user to submit a new request. Any violation of any of the terms and conditions of this agreement shall provide justifiable cause for the immediate termination of the agreement. I agree to supervise and hold the district harmless for any injury or accident, which occurs during our group’s use of the school facility, and to indemnify and hold the district harmless from any injury or accident, which may occur.
