This Support Agreement (“AGREEMENT”), effective as of September 30, 2018 (“Effective Date”) is entered into by and between Managed Access to Child Health, Inc. d/b/a Partnership for Child Health (“THE PARTNERSHIP”), a Florida not-for profit organization located at 910 North Jefferson Street, Jacksonville, Florida 32209 and I. M. Sulzbacher Center for the Homeless, Inc. located at 611 East Adams Street, Jacksonville, FL 32202 (“SULZBACHER”). SULZBACHER and THE PARTNERSHIP are collectively referred to herein as “Parties.”
