I, , will be attending the American Conference for Irish Studies during the period March 30 through April 3, 2016 at the University of Notre Dame, Notre Dame, Indiana (the “University”). While attending the Conference, I have requested, as the parent or legal guardian of a minor child, to leave my minor child, , with Conference volunteers in McKenna Hall in order to utilize babysitting services (the “Services”) offered by the University. I am fully aware that my child’s utilization of these Services is totally voluntary.
