If you are a resident of Alberta, Manitoba, Northwest Territories, Nunavut, Prince Edward Island, Yukon or Saskatchewan and you are subscribing for more than $10,000 in Units, complete and sign a Declaration of Eligible Investor Status – Appendix B – unless you are an "accredited investor" (in which case you must fill out the Appendices referred to below) or are not an individual and are purchasing at least $150,000 of Units.
