This Services Agreement (this "Agreement") is effective as of February 1, 2021 (the "Effective Date") and is entered into by and between the Board of Education of the City of Chicago, a body politic and corporate, commonly known as Chicago Public Schools, with offices located at 42 West Madison Street, Chicago, Illinois 60602 (the "Board" or “CPS”) and DANIELS SHARPSMART, INC. dba DANIELS HEALTH with its principal place of business located at 111 W Jackson Boulevard Suite 1900 Chicago Illinois 60604 (“Vendor” or “Daniels”). The Board and Vendor shall be referred to each as a “Party” and collectively the “Parties.”
