The Physical Health Managed Care Organization (PH MCO) has entered into a an agreement dated (the "HealthChoices Agreement") with the Department of Human Services of the Commonwealth of Pennsylvania (“Commonwealth”). Under the HealthChoices Agreement, the PH MCO has agreed to provide certain supplies, services or construction (“Services”) to the Commonwealth.
