I, the above‐named Participant, am eighteen (18) years of age or older. In consideration of the opportunity to use the LSC‐ UP Fitness Center, I hereby assume all risks of injury, illness, death or other loss arising from or in any way relating to my use of the LSC‐UP Fitness Center. I acknowledge that my use the LSC‐UP Fitness Center may expose me to hazards or risks that may result in my illness, personal injury, or death, and I understand and appreciate the nature of such hazards and risks. I understand it is my responsibility to consult with my physician regarding my physical activity and use of the LSC‐UP Fitness Center, and I represent that I am physically able to use the LSC‐UP Fitness Center.
